This template gives you the columns most providers converge on for tracking credentials, the tickets, checks and licences a third party issued rather than training your organisation delivered. It does not decide which credentials your organisation requires for a role, and it is not a screening or credentialing policy. Those decisions stay entirely yours.

At minimum: worker name, the credential name and category, who issued it, the date issued, and the expiry date where one applies. The NDIS Worker Screening Check clearance and a worker's qualifications and experience for the role are two specific artefacts an assessor asks for by name.
No. A credential register tracks externally issued tickets, checks and licences and their expiry dates, things a third party issued. A training register tracks courses your own organisation assigned and delivered. A worker can be current on one and lapsed on the other.
No. This is a practice tool for tracking credentials your workforce already holds. It does not set which credentials your organisation requires for a role. Those decisions, and your screening and credentialing policy, remain entirely your own.
