This is a practice tool, not a policy document
This template gives you the working set of columns most NDIS providers converge on and most auditors expect to see. It is not a training policy, and it does not decide what training is mandatory for your workforce. The NDIS Practice Standards do not prescribe a register template or a fixed list of fields. What they require is that a provider can demonstrate its workers are appropriately skilled and trained for their roles, and that this is documented. That decision, and your training policy, stay entirely yours.

Delivery method distinguishes an online module a worker read from an in-person or observed session, which is usually the field a completion export leaves out. Delivered by is your own team, an RTO, or a platform. Leave expiry blank for training that does not carry one.

At minimum: worker name and role, the course or training activity, who delivered it, the date completed, a certificate or record reference, an expiry or renewal date where one applies, and the delivery method (online, in person or external). Miss the role and you cannot show the training suited the job. Miss the delivery method and an online module looks identical to an observed practical session.
