This is a practice tool, not a policy document
This template gives a supervisor a shape for the conversation. It is not a supervision policy, and it does not set how often supervision should happen in your organisation. The NDIS Practice Standards set no minimum frequency for supervision. What an auditor looks for is a documented cadence you have set, and records showing it was kept. That decision, and your supervision policy itself, stay entirely yours.
Adapt the sections below to match how your service actually runs supervision. Add a section, drop one, change the language. It is yours the moment you print it.
NDIS Support Worker Supervision Agenda
1. Check-in
2. Review of previous actions
3. Practice discussion
4. Training and development
5. Feedback, both ways
6. Agreed actions and next review
Where this lives if you want it held digitally
CORA holds supervision digitally: sessions on the worker file, the register kept current without a spreadsheet. One complete course is also free: Professional Boundaries & Ethics.

The NDIS Practice Standards do not set a minimum frequency. What an auditor expects is a documented cadence the provider has set, and records showing it was kept. Many providers run more frequent sessions in a worker's first months and space them out from there, but that is a provider decision, not a fixed NDIS rule.
No. This is a practice tool: a blank agenda a supervisor can print or adapt. It is not a supervision policy and does not set your organisation's supervision requirements. Your supervision policy, including cadence and who is responsible, remains entirely your own.
Yes. It is a starting point. Add or remove sections to match how your service actually runs supervision.
CORA holds a supervision register with the cadence set per employment type, sessions built from combinable parts (a conversation, a Support Observation, a performance review), and an evidence pack that prints any date range. See NDIS supervision records for the full detail.
